A Resident Lifestyle Profile is a collection of information which allows you to “know” the resident, their history, their family background, their spiritual and cultural background, their likes and dislikes, hobbies and much more. This information is necessary to complete a comprehensive care plan.
We have created an interactive profile that can be filled in on your computer before printing.
IMPORTANT: Once printed the document needs to be Saved As into your Resident files on your computer. If you close the file without saving information will be lost.

Each resident has a participation record of activities he or she has attended and to what level they participated in an activity.

Resident of the Day Form
Most facilities will have their own Resident of the Day records, however if you find yourself needing one please download ours for your use.
This form can be used to check all aspects of a resident’s care are being met and changed if needed. Most are reviewed on a monthly basis.
This form is not interactive and will need to be printed and filled in manually.

Hi, My team are seeking a leaflet that has suggested wording to assist with documentation, whilst keeping to the new standard requirements.
Where you are writing in the first person, as opposed to it sounding like the staff member making the observation.
Eg;
Mary has previously enjoyed weekly tennis matches with friends in the local area, assisting her to keep fit.
When I was at home I regularly attended tennis with my friends, this allowed me to keep fit and catch up with friends each Friday morning.

Wow, wow, Wow!!!! I love the assessments and wished I had found these beforehand. I have been batteling with Autumncare which is very lengthy and difficult to use for interviewing residents. So this is soooooo brilliant. You have done it again Birgit!!! Tops, tops, tops!!!

Can you tell me if you have an assessment tool to assess theclient/resident before they begin an activity able to cut with scissors, able to use a needle and thread and able to use knitting needles. able to use a knife in cooking groups
Able to pick up a bingo to

Able to read……….etc and more assessments
I would hate to set a client up to fail.
